Are you interested in applying for long term services? Long term services allow you to access a variety of supports through the county. You may have access to Brokerage services, long term job coaching, 1:1 attendant care or DSA supports. To find out more about long term supports, and brokerages supports, click here.
Applying for services can seem daunting. It takes months to get a decision and it can require you to provide more and more information in order to be approved. We at BCM are happy to assist in any way we can!
Your first step is to apply for Medicaid. If you are an adult living in the community who has been diagnosed with an intellectual or developmental disability, you may qualify for these services. Below are links to both the Medicaid website and a direct link to the application page:
Once you are approved for Medicaid, you will want to make an appointment with DHS to get set up with a Service Coordinator. Below is the link to the DHS Website. You want to fill out the referral form with the link provided on that page:
Once you are placed with a Service Coordinator, they can assist you in setting up long term services.
